This form must be submitted by all applicants intending to study while on student or scholar visas. The form along with the required supporting documents provides us with the information required to prepare your I-20. This form should also be used by current GGU students who will apply to an additional degree program upon completion of their current academic program.

The Institutional TOEFL test is administered six times a year at GGU. In order to register for the exam, you will need to submit this form to IAA Services in person, by mail, or by fax.

Curricular Practical Training (CPT) Packet (pdf)

F-1 students may be eligible for off-campus work authorization due to economic hardship if the need to work is caused by unforeseen circumstances not within the student's control. Examples of this include substantial fluctuations in the value of currency or in the exchange rate, loss of financial aid, unusually high medical bills, or other unexpected circumstances. This form addresses frequently asked questions and provides instructions on the application process.

Students who would like to enroll in less than a full course of study should submit this form to IAA Services at the beginning of the term. You will need to submit this form if you would like to take a vacation term, if you need to take less than full time due to a medical condition or if you are in your last semester and need less than a full course of study to complete your degree. If you need to request a reduced course load due to academic difficulties, please make an appointment to see your counselor. International students must receive permission from their counselor before dropping below full time in order to maintain their immigration status.

If you already have comparable health insurance coverage, you may apply for a waiver of GGU's health insurance plan. Please bring the waiver form and supporting documentation to IAA Services. We will review your waiver at the time of submission, so that your registration hold may be lifted.

Please use this form to request a travel signature or to replace a lost or stolen I-20. In order to obtain a travel signature, you will need to submit this form along with your most current I-20. Please allow International Admissions and Advising Services five business days to process your request.

This form should be submitted if you need an official letter from IAA. Frequent types of letter request include verification of immigration status, verification of enrollment for visa renewal, graduation invitations for family members living outside of the US, and verification of status for Social Security Number applications.

Please submit this form to International Admissions and Advising Services if you need to transfer your SEVIS record from GGU to another academic institution. You should submit a copy of your admission letter with this form. We may only process one request to transfer, so please be certain that you will attend the requested institution.

If you plan to apply for a J-1 visa and to request a DS-2019 through Golden Gate University, you will need to submit this form. The J-1 visa category is open to exchange visitors or scholars who are primarily sponsored by the U.S. Government, your home country government, or an international organization of which the US is a member by treaty or statute.

If you need to extend the program end date on your I-20, you will need to submit this form. You should contact your international student counselor to discuss your situation. Your counselor will be able to assist you with the process.
